Abdominoplasty: tummy tuck, abdominal surgery

Correction of a damaged abdomen is generally performed with a scar above the pubis. It is possible to remove skin and fat and to tighten the muscles. Sometimes liposuction alone is enough, when the skin is elastic and the muscles are not very stretched.

A wide variety of situations may have prompted your consultation: significant excess skin, stretch marks after repeated pregnancies, a firm but overly round abdomen with excess fat, or an abdomen that protrudes forward after a few pregnancies even though there is no significant excess of fat or skin.

Analysis of the deformity

The following must therefore be assessed:

  • the patient’s posture and spine,
  • the muscle tone of the abdominal wall, looking for any separation (diastasis recti) seen after pregnancies,
  • the intra-abdominal volume,
  • the fat layer,
  • the quantity and quality of the skin.

Possible operations

  • simple liposuction in the case of isolated excess fat, through 1 or 2 tiny incisions. Moderate excess skin will retract.
  • a mini abdominoplasty, which generally combines liposuction with removal of the excess skin above the pubis only, which amounts to adding a scar (fairly low) just above the pubis.
  • a classic abdominoplasty, which consists of removing most of the excess skin by pulling it downwards. A long horizontal scar extending into the groin fold on each side of the pubis is unavoidable, sometimes combined with a short vertical scar. There is also a scar around the navel, which stays in place. Liposuction is often combined with the abdominoplasty for the upper abdomen, and the abdominal muscles are corrected if necessary.

Recovery

Moderate activity should be planned after the procedure. If the muscles have been brought together, some movements will be painful during the first fortnight. Compression in the form of a special elastic girdle is very useful, worn first day and night (15 days to 3 weeks), then during the day (for 4 to 6 weeks). Sport is prohibited for 2 months after muscle repair.

Anticoagulant prophylaxis is indicated for several days after the operation in almost all cases; this prescription aims to significantly reduce the risk of phlebitis.

Evolution

A little patience will always be needed to appreciate the result, as it will take 2 to 4 months for the swelling to subside completely.

If an abdominoplasty with a long scar was necessary, the scar will fade very gradually (1 to 2 years) but will almost always remain slightly visible.
If an irregularity persists after liposuction, a limited touch-up will almost always be possible. In the case of muscle repair, sport must not be resumed before six full weeks.
