With the vertical technique, it is possible to correct almost any sagging breasts, whether they are too large or, on the contrary, deflated. The procedure causes very little pain, and in most cases avoids the long horizontal scar under the breast. The vertical technique involves only an incision around the areola and a vertical one. The breast regains its shape within a few months.

Sagging and/or excess breast volume
Breast ptosis and/or hypertrophy.
In both cases, a certain amount of skin will have to be removed. When the gland is too large, the necessary amount will be removed; when, on the contrary, it is flattened and spread out, it will be gathered together.
In both cases, I will rebuild a high, round mammary gland. It will most often be temporarily anchored to the pectoral muscle, and wide arm movements are not advisable for a few days. The procedure will of course be performed under general anaesthesia, and one to three days at the clinic will be necessary.
If you are concerned about the possible effects of surgery on the mammary gland, it is important to know that current detection methods make it possible to distinguish between possible small post-surgical glandular calcifications and images suspicious of a tumour. After a clinical breast examination, it is essential from a certain age to have a pre-operative mammogram and ultrasound to make sure that everything is normal.
During the procedure, the state of the mammary gland can be examined more closely, and a cyst or suspicious fragment can be sampled, the removed fragment being examined by the pathologist under the microscope.
Very large studies have been carried out to determine whether surgery could promote the later development of breast cancer. They have always concluded that it does not, pointing out on the contrary that a number of small early cancers have been discovered or removed thanks to the cosmetic operation.
Reducing the volume of a large gland also proportionally reduces the risk of a later cancer, not to mention the fact that the constant pulling of overly heavy breasts is in itself a factor of irritation for the gland.
Overly heavy breasts also often cause poor posture and back pain, in addition to the difficulty of finding clothes. There is therefore less reason to worry about scars in this case, as the aesthetic benefit will in any case be significant.
The question of scars regains its importance in less severe cases, particularly minor ptosis, slight sagging of the breasts: the pros and cons must be weighed, and sometimes the operation should be forgone.
You will always have a scar around the areola, whose size I will reduce if it is stretched. Below the areola, the scar will be either vertical or in an inverted T (vertical downwards from the areola, and horizontal in the fold under the breast). When reducing very large breasts, the scars have to be long. On the other hand, if the breasts are not very large, a vertical-technique scar (I-shaped) is possible. The final result only begins to be appreciated two to three months after the procedure, sometimes even 6 months.
Even if the scar does not evolve perfectly, it can at least be hidden without difficulty. In any case, scars located on the breast usually improve a great deal with time, whereas scars extended in the middle and to the sides may remain very visible. As one has to “cheat” to achieve this vertical scar, a small fold sometimes remains, which will be corrected six months later, if necessary, under local anaesthesia.
In some cases, the scar can be limited to a vertical one, and sometimes even to a scar around the areola only.
For ptosis corrections without implants and for breast reductions, pain is moderate and responds well to the usual painkillers. There is some discomfort when the drains are removed the day after the operation. A sports bra must be worn day and night for two months, except for showers, which can be taken from the second day after the operation. Of course, sports involving the arms are prohibited for two months.
